my yamaha250 timberwolf keeps poping out of gear. please help!!

i have a 96 yamaha 250cc timberwolf. iv had for about 2 years and changed the oil maby once or twice. but now the oil level keeps droping down really low. it also pops out of gear and makes a loud grinding noise as i dont go enywhere. after about 5 secounds of the grinding it pops back into gear and i keep riding. it happens freakwently while i ride it. i try to keep adding yamalube but it doesent make much of a difrence. please comment and tell me what could be wrong it.
